Personal Accident Claims Procedure
1. If a serious or fatal accident occurred, please give immediate notice to us. Loss Adjusters may be appointed to investigate the matter.
2. Fill in the Personal Accident claim form and pass to us with the following documents within 14 days of the occurrence of the accident.
a. Accident investigation report, if any
b. Death Certificate and Coroner's Report for fatal case. Insurer may request for post-mortem examination at their expenses.
c. Medical report, if any
d. Sick Leave Certificates and/or Medical Certificates
e. Original medical receipts en the employer & employee (if applicable)
f. Form 5 & Form 7, if applicable
3. Insurer may request the insured personal for all medical examinations from time to time as required and arranged at their own expenses
